There are 4 electrical leads, and one SMA RF connector. Sorry, we do not know pinout info and specification. Please check manufacture test data at the last pdf link below and do your own research on how to debug and how to use it. We tested resistance between any two pins from either side (by multimter, cascading 2.2 KOHM resistor to limit current), only two pins near fiber shows 102 KOHM resistance from either side, any other two pins from either side is unlimited large. The resistance bewteen any pin and metal case is also unlimited large. The manufacture test data sheet in last pdf link below only shows one bias voltage, so probably PD bias and Amp bias share same volatge, probably you only need add bias on the two pins near fiber.
