There is a label showing "FO MOD 2.5GBPS CBAND PM/SW BIASFREE" in last photo below, so we believe it is phase modulator. Without fiber connector. With 3 electrical pins for RF. We do not have pdf specification. You need try by yourself to figure out how to use it.
