JDS PS3 Optics assembly kit P/N: ED011894-A-03. It includes 1x3 JDSU PS3650+25 SW13-Z349 non-latching SWITCH with 3 fiber connectors, 2 APD detectors, one 2x2 splitter with one FC/APC connector, and one optics block.

We figured out optical configuration of this kit, see 3rd photo below. See 5-page manufacturer test data at last pdf link below. All fibers are 900um buffered.

You can buy whole kit or buy following parts individually:
• Whole kit.
• 1x3 optical switch SW13-Z349: For both 1.55um and 1.3um. See general specification of optical switch at 1st pdf link below. Direct or TTL control of switching. Optical fiber of input port is only 20cm long, there are two FC/PC connectors and one FC/APC connector at 3 output ports. With manufacture test data at last pdf link.
• AC1100 2x2 3dB coupler over both 1.55um and 1.3um, port 1: with FC/APC connector, ports 2, 3, 4 without connectors: 15.5cm fiber, 15.5cm fiber, 33cm fiber. With manufacture test data at last pdf link. See specification at 2nd pdf link.
• APD detector: with 15.5cm fiber without connector. With manufacture test data label. The last two photos below are for the manufacture test data of APD detector (responsivity, breakdown voltage, dark current).
• APD detector: with 27.5cm fiber without connector. With manufacture test data label.
