10Gb optic intensity modulator with dual RF drives (looks GPO connectors), Z-cut. The 2-pins are for internal tap-monitor(-5V bias). With FC/PC connector at the fiber output end(SMF-28), with FC/APC connector at the input fiber end(PMF, Fujitsu Panda SM-15-P-8/125-UV/UV-400). "Unqualified prototype" is marked on the modulator body. It seems not meet Lucent (customer) system requirement, the test sheet shows the “RF(2) S11 (0-10GHz)” and “Main Loss” Fail. But it would be still OK for your research. Please review the photos below and make decision by yourself.
