100GHz-to-50GHz C-band interleaver. With 3 LC/PC connectors. See photos belows from actual module for more info.

We verified that Pin3: RTDB1 and Pin4: RTDB2 are internally connected. So the pinout is same as the 1st pdf link below. Basically you need connect two heater pins and two RTD tempearture-sensor pins to your temperature controller (many types of temperature controllers support RTD temperature sensor). Then you can easily set interleaver to required temperature (71.7 C).
