Model: OPF 693-2, OPF 3172, or OPF 3175, and 1pc unknown model. We believe they are similiar to model: 372A. Tested output with 50/125um fiber jumper: -18.0dBm to -25.0dBm at 100mA. 3-pin. Metal housing. The output is few dB lower than specification. The photo and the datasheet in the pdf link below are for model 372A only.
