It is type-2 pinout, STL5412 pinout should be exactly same as SLT5410, SLT5416. We use E-tek laser controller to test samples of SLT5412 (and also SLT5410, SLT5416, and also almost all other signal lasers). See E-tek laser controller pinout at the last photo.

Sorry, no datasheet for SLT5412. SLT5410, SLT5412, SLT5414, SLT5416 are designed for direct modulation with SMF. The output power of SLT5412-xB is 20mW at “1” code of signal (Ppeak) and 10mW of average of “1” code and “0” code (Pave). If you use it for CW mode(continuous wave), it would support 20mW (continuous “1” code) at worst case, but the manufacturer test sheet normally only gives data at average power 10mW. The life time may change if you use as 20mW at CW mode, see reliability report at last pdf link below.SLT5411, SLT5413 are designed for external modulation with PMF, so normal lasing condition is CW mode (continuous wave).
