It was removed and desoldered from unused long-reach 10Gb transponder subsystem. The photos are before removing modulator !
It was inside unused 9.953Gb -11.096Gb transponder subsystem (see 1st photo), see specification of this transponder at last pdf link below, it is with negative chirp. Single-drive model FTM7924 offers fixed +/-0.7 chirp parameter, as shown at 2nd pdf link below, which allows negative chirp, and allows up to 12.5Gb modulation rate. 'H74M-5209-J037' is marked on modulator case. The top image of 3rd pdf link below for FTM7924FN also shows H74M-5209. ‘FJ’ package of FTM7924FJ matches the dimension of this modulator. After we compare all Fujitsu modulator models FTM792(0,1,2,3,4,6,8)(EB,ER,FB,FJ,FN), we conclude this modulator model is FTM7924FJ.
900um-buffered fibers. Without fiber connector at modulator input. With SC/APC fiber SMF connector at modulator output (SMF verified under fiber scope).
With one GPO RF connector, GPO/SMP male interface. You can use GPO(SMP)-female to SMA-female adapter (Store part #: EIP-RFADAPTERS-017D) to convert to SMA interface. SMA can work up to 18GHz. There is one GPO-femal to GPO-female RF jumper, connected to modulator RF port.
With 4 pins,
G: GND
B: DC Bias
A: PD Anode
C: PD Cathode
