2 SMA RF connectors. Bias: a few V. RF: in the range of 5V. by MICRO PHOTONIX Inc. There are two connectors were fusion-spliced on the both fiber ends by previous tester. The connectors look SMF (rather than PMF) under fiber scope, so you may need fusion-splice PMF connector at the input fiber for standard telecom application.
