The product model: FLD5F7CZ/059W21 is marked on the product box (see 1st photo below), but this product box is not original. Since we got them from same supplier of FLD5F7CZ/059Wxx and 10mW FLD5F7CZ201WxxP, so believe this laser is also FLD5F7CZ/059Wxx and 10mW FLD5F7CZ201WxxP. We very believe it is 10mW laser since we got them from 10mW analog transmitter system makers. DFB DWDM Direct Modulation Laser. 14-pin butterfly. SMF with SC/APC connector. Without test data. It seems very similar to FLD5F7CZ-J. You may find more info for similar-mode datasheet at the pdf link below. Made in Japan.
