1x2 JDSU Switch with 3 Panda PM fiber FC/PC connectors. JDS P/N: SW12PM-Z16

We verified Panda PM fiber at each optical port under fiber scope, PM Axis alignment aligned to the slow axis. With 900um buffer fibers. There is one green electrical pin, and one red electrical pin. We believe this SW12PM is same as SW12 model in 1st pdf link below, but fibers are PM, so you can simply apply 5V DC power supply to switch optical path (connect green pin to ground).
