We believe it is used as booster amplifier, based on part number "OUTH". It includes erbium-doped fiber, three JDS isolators (IS3550), WDM couplers, JDS VCB-Z001D electrical-controlled optical attenuator module, two photodiodes, tap couplers, three passive gain-flat filter devices(RF1558-TL-NJ1, RF1546-TL-NJ1, RF1533-TL-NJ1). Two external pump lasers are required. Sorry we do not know the pinout, so you may have to use this module as parts. Made for Tellabs.We opened the sealed module case, only to see what is inside.The test sheet in last three photos below are from similar models OA1510-ANS2 and OA1530-32-IN, only for reference.
